This paper describes a major review, carried out by Shell UK Exploration and Production, of its Permit To Work System. The key elements were seen to be:

  • Concentrating management attention on work of high potential risk by specifying commensurate authorisation levels.

  • Co-ordination of non-routine activities and providing visibility of the status of work in progress.

  • Clear system specification.

  • Disciplined isolation procedures.

  • Constantly updated computer based permit information and tracking system.

  • Training.

  • Auditing.
