Incident investigation and analysis for E&P operations is part of performance monitoring in health, safety and environment (HSE) management systems. To understand how to improve HSE performance & which managerial aspects have to be changed, it is essential to have a meaningful system for assessing the underlying causes of incidents. Prevention of incidents is a primary objective of HSE management, and continuo reduction of the risks of major accidents requires an understanding of the potential hazards present, the reasons for occurrence, and the extent to which situations could occur across other locations & operations.
