This system was designed for small plants, but is scalable upward. The Standard Operating Procedure (SOP) and forms for this system are included at the end of this proceeding paper.

Conceptually the following people are needed:

  • A single point of entrance – generally a security guard, but could be a lobby with a phone

  • Site Contacts – People who manage contractors and issue permits (Maintenance Manager, Supervisors, etc.; EHS Manager can handle this, but is generally a backup)

  • Auditor – generally the EHS Manager who audits the contractors and handles pre-qualifications

SOPs and Forms needed are:

  • Contractor Safety Management SOP

  • Hazardous Work Permit SOP

  • Life Critical SOPs – Hot Work, Confined Space, Lock Out/Tag Out (LOTO), Fall Protection

  • Electrical Safety in the Workplace (NFPA 70-E)

Tracking and or records needed:

  • Contractor Training

  • Certificate of Insurance

  • Pre, During and Post Audits

  • Completed Pre-Qualification Determinations

  • Permits

  • Contractor OSHA Log (if PSM Covered)
