Introduction

The title of this paper may lead one to think that this is just another of those catchy phrases that gets tossed about from time to time within the safety community. Much like "safety is no accident," or "safety is job one," we use phrases to help illustrate concepts regarding the safety practice. However, if the underlying premise of this title is an accurate one, this might be an issue that should get all of our attentions and perhaps motivate us to begin to develop some solutions to what is a major issue facing the safety profession. So what is meant by the phrase Safety By Accident? This paper will explore these topics and hopefully cause one to think more deeply into what can and should be done to make those necessary changes.
